Job Summary:

Allevio Care Anywhere is seeking a dedicated and skilled In-Home Physical Therapist with wound care certification to join our growing mobile care team. This role specializes in delivering hands-on, patient-centered wound care services in the home setting with a focus on advanced wound management. The ideal candidate has experience with Ultramist® Therapy, is proficient in applying graded compression wraps, and is comfortable executing physician-directed wound care orders.

Key Responsibilities:

• Collaborate with interdisciplinary teams (DPMs and NPs) to ensure integrated care delivery
• Administer Ultramist® Therapy for chronic and acute wounds, following established clinical protocols
• Apply and monitor graded compression wraps to treat venous insufficiency and other lower extremity conditions
• Implement and follow through with physician-prescribed wound care orders
• Educate patients and caregivers on wound prevention, proper dressing care, physical mobility strategies, and functional goals
• Maintain thorough, timely, and accurate documentation using the Allevio EMR system
• Adhere to infection control protocols and safety practices during all home visits
• Monitor wound progression and provide updates to supervising clinicians or referring providers as needed

Required Qualifications:

• Current Physical Therapist (PT) license in the state of Utah
• Wound care certification (e.g., WCC, CWCS, or equivalent) required
• Experience with Ultramist® Therapy preferred
• Skilled in placing and managing graded compression wraps
• Previous home health or mobile therapy experience strongly preferred
• Excellent clinical judgment, communication, and independent problem-solving skills
• CPR/BLS certification required
• Valid driver’s license, reliable transportation, and current auto insurance

Schedule & Work Environment:

• Flexible scheduling options available – part-time or full-time
• Field-based position with travel to patient homes
• May require lifting or assisting patients and equipment up to 50 lbs
